Jewish Hospice Program
The Jewish Hospice Program provides emotional, spiritual, financial/budgetary and practical support to individuals and their family members. Our team helps clients navigate the healthcare system and works alongside their care providers to offer integrated and collaborative care, within a client’s means and cultural values. The Jewish Hospice Program is staffed by a Program Manager, Hospice Social Workers, Rabbi Chaplains, and a Volunteer Manager (through Circle of Care).
The Jewish Hospice Program provides:

Counselling

Care planning, resource identification, advocacy and system navigation

Spiritual support

Support for caregivers and caregiver relief

Support for individuals, families and children

Grief Counselling and Bereavement support

Trained volunteers (in partnership with Circle of Care)

Connection to a range of JF&CS services including financial assistance (where applicable)

Services available in English and Russian
General Eligibility (please call for more information):

Those diagnosed with progressive life limiting illness who identify as Jewish and their families

Catchment: from Lakeshore to Bloomington Road, 400 to the 404 (most of Greater Toronto).
The Hospice program is available at no cost to families who identify as Jewish.

General Eligibility (please call for more information):
Individual Support: Open to those in the GTA who identify as part of the Jewish community and have experienced a recent loss (within the last 24 months).
Bereavement Groups: Aimed at the Jewish community but open to all (no limit on the length of time since the loss).

Education and Consultation
The Hospice, Bereavement and Grief team is available to support professionals in social service and healthcare settings by sharing expertise and providing education about Jewish cultural and spiritual needs at end-of-life and into bereavement. We provide seminars and consultation with a cultural and clinical perspective.
