Transitional Worker

Full time | September-June (10 months); 35 hours per week

Salary Range: $65,000 to 77,000 based on experience

The Jerome D. Diamond (JDD) Adolescent Centre, a branch of Jewish Family & Child Service is looking for a Transitional Worker to provide direct services, within an effective treatment program, to adolescents during the school year at the JDD Adolescent Centre. The JDD Adolescent Centre is a children’s mental health centre that provides support to youth with educational, emotional and/or behavioural challenges. The Transitional Worker provides counselling support and treatment to selected adolescents, either currently attending or having graduated from the JDD.

 

The successful candidate will:​​

  • Provide direct and relevant services to selected students who struggle with social, emotional, behavioural and/or learning difficulties in conjunction with the JDD team.

  • Counsel and coach selected youths on a regular basis.

  • Develop, implement, review and monitor the treatment plan for the youth in conjunction with the team and family.

  • Plan and participate in activity/recreation based programing and physical education such as yoga, art therapy, cooking, CBT etc.

  • Supervise and assists students during non-academic instructional time, e.g., recess, lunch, bus loading/unloading, therapeutic recreation, etc.

  • Ensure appropriate transportation for students as needed; this may include bus schedules, taxis, and parent calls for pickups

  • Assist the CYW(s) in completing case reports, daily logs and other related documentation as needed.

  • Prepare various JDD related documentation such as information packages, admission documentation, back to school packages, holiday lists, etc.

  • Maintain accurate and timely client related documentation for internal and external record keeping in accordance with professional standards, the Agency and Day Treatment Policy.

  • Ensure the health and safety of all staff and student by adopting and adhering to health and safety guidelines.

  • Perform other duties as assigned.


Qualifications:

  • CYW or equivalent post-secondary diploma/degree

  • 3 years’ experience as a CYW in in a Day Treatment Program 

  • Knowledge of community needs and resources 

  • Ability to work effectively in a multi-disciplinary team

  • Strong administrative and organizational skills

  • Strong computer skills and within with Microsoft Office Package (Outlook, Excel and Word), databases, and client databases.

  • Comprehension of French or another language will be considered an asset
